Doctors or Specialists who wish to refer patients can complete the form below.
For your convenience, we recommend that you:
- open the referral form (PDF format)
- complete your details and forward to email below.
- save the PDF for future use (as required)
- you are welcome to email this to info@curatdhealth.com.au or call us us on 07 3180 6750 to make an appointment.
- Please email a referral to info@curatdhealth.com.au including your and your patient’s contact details.
